In what way did the card fail? Was is gradually or did it fail suddenly?

Also - have previous repairs been attempted to the card?

One day I turned the pc on and there was no display and the day before was perfect. The card was recalled a few years ago by the manufacturer to replace a heatsink or something along those lines. It's also been taken apart and reassembled.
