Chassis:
CFI A7879 Mini-ITX Server - Home Server / NAS chassis with lockable and hot-swappable 3.5" SATA drive trays and built-in 200W Flex-ATX power supply

PSU:
See above - built into chassis and works fine

Motherboard:
Asus E45M1-I DELUXE with integrated WiFi, E-450 CPU and Radeon HD 6320 GPU

RAM:
1 x 4GB DDR3 Kingston thing

Disk:
320GB 2.5" SATA drive with Ubuntu installed on it

I still have the motherboard box, manual and driver CD - some ancillary items like wifi antennas that came with the motherboard - the basic locking keys for the chassis and that is about it. It would make a good Linux based NAS or an awesome... um... oh I have no idea what other applications you might want to use it for, but it is a nice and neat little machine. Check out the case dimensions. It is pretty tiny. Obviously the sad part is that I am not including any drives in the hot-swappable bays and it only comes with the drive that I previously used as an OS drive.
